About the resource: The Slough Assertive Outreach Team acknowledged that there was a severe gap in the service regarding the quality of physical health, after it was recognised that 90% of their 55 service users suffered from a multitude of conditions, including obesity, diabetes, high cholesterol, respiratory disease or heart disease.
Maintaining a healthy lifestyle with good nutrition and physical exercise is challenging for anyone, but healthy eating and exercise education offered through a group setting enables service users to share ideas, skills, common problems and issues.
This pack and the accompanying materials can be used and adapted to suit all purposes, whether your plans are a temporary short-term group, long-term continuous group, in an inpatient setting or a community setting.
